Services We Provide for Participants

With our hourly services, we provide services for individuals who are able to live more independently but still need help throughout the week. We assist with daily living skills, appointments, medication support, community activities, household responsibilities, and other goals identified in each individual's service plan. 

For high support and group services, we work with multiple participants and provide subsidized living. Through these services, participants learn how to live with others, and be involved and engaged in the community through their  daily structure and routines. 

We provided one-one-one support for individuals who require a higher level of ongoing support due to significant behavioral, safety, or daily living needs. Our team provides structured individualized support focused on safety, stability, independence, skill development, and behavioral goals. 

We also support individuals who are transitioning back into the community after incarceration or other institutional settings. Our goal is to help individuals successfully rebuild their daily lives by providing structure, skill development, and support as they adjust to living in the community.
